The St. Elizabeth College of Nursing Alumni Association Recognition Award

Each year, at the Alumni Banquet held the last Friday of April, we honor our alumni. This life time achievement award, is given in recognition of the alumnus with outstanding qualities such as leadership, education, professional achievements, community service and contributions to the St. Elizabeth College of Nursing and the Alumni Association from their school days throughout their lives. 

The areas on the form below do not all have to be completed if the nominee does not have those qualifications. This does not mean that they will not be considered for the award. These candidates do not have to have the higher education degrees. This form provides guidelines and thought provoking questions for those nominating someone.  

Anyone can nominate another for one of these awards. You may nominate as many people as you choose. We ask that you follow the guidelines and submit the form in a timely manor, as indicated on the form.
